This information is used to complete the death certificate and permit, and to notify Social Security. All provided information must be accurate.

The legal next-of-kin is required to sign, authorizing the Cremation.

 

Note: only the person or persons in each category listed below is required to sign. To determine next-of-kin, the categories below must be followed in descending order. Categories cannot be skipped if there are living members in the category, unless they are incapacitated or their whereabouts are unknown. In either case, documentation will be required attesting to their circumstances. Contact our office for help regarding this situation.

 

The descending order of legal next-of-kin is as follows...

  • Spouse of deceased
  • All children over the age of 18
  • Custodial parent
  • Non-custodial parent

If you are Pre-Planning you may NOT choose Credit/Debit Card as a payment option. 

Thank you for placing your trust in us for your funeral service needs. We understand that making financial decisions after the loss of a loved one can be difficult. We encourage you to ask questions and have an open discussion with your director regarding payment options so that you can make the best decision for your family.

 

For your peace of mind, until a method of payment has been secured and/or an insurance policy assignment been confirmed, we will not proceed with scheduling the visitation and services or running a public obituary notice. Once payment and/or insurance confirmation has been secured, we will proceed with all of your arrangements as described in this plan.
